For those of us at Community Foot Specialists, our patients are more than just feet. We hope our patients look to us as more than just their foot doctor. We strive to create a welcoming atmosphere in each of our six locations throughout the Dayton and Springfield area. You won’t have to wait at a closed window for our front desk staff to get off of the phone - we are there to welcome you when you first set foot into our office. And while we don’t expect to keep you waiting long for your appointment, we offer coffee and water to help make your wait more comfortable. Our doctors believe in a hands-on treatment approach. We aim to provide our patients with the highest quality care available, delivered effectively and efficiently, while educating patients on their particular condition.
By clicking on the above link, you will be able to download and print all of the forms you will need for your first visit to our podiatric office. Please print, and fill them in at your convenience prior to your first visit to our office. If you are unable to print them at home, please arrive 30 minutes earlier than your appointment time to complete them in our office. Along with these forms, you will need to bring a photo ID, your insurance card/s and a list of medications if you have one.
